Instructions for completing the DS-156 online
• Visa applicants are required to complete the DS-156 online. Handwritten forms will NOT be accepted.
• All answers must be typed in English using English characters.
• The information in the passport must match the information on the application.
• All questions must be answered – if the question does not apply to you, please enter “Not Applicable.” Do not leave any fields blank.
• You must print out the form when you are finished and be sure that you have all three pages, including the barcode. Your application can’t be processed without the barcode.
