Social Security Numbers/Cards
Applications for new or replacement Social Security numbers and/or cards can be submitted through the Consular Section at the U.S. Embassy in Kabul. Form SS-5-FS must be used.
The applicant should complete the form and supply proof of age, as well as citizenship and identity evidence. For minor applicants, proof of identity for the signing parent must also be presented. Most commonly, a birth certificate and unexpired U.S. passport are used to prove age, citizenship, and identity. Please refer to the instructions on the form SS-5-FS for a list of other possible identity documents.

Medicare Enrollment
Medicare enrollment can be arranged via the Federal Benefits Unit at the U.S. Embassy in Athens, Greece (this is a regional office that covers Central/South Asia). Please send an e-mail to usconsulkabul@state.gov containing the following information:
- Full name
- Birthdate
- Social Security Number
- Local telephone number
- Local address
- U.S. address/permanent address
This information will be forwarded to the FBU in Athens, and an individual from that office will contact you directly to complete the enrollment process.
